What does this mean? Unlike patients who dislike their large chin or feel it is disproportionate, patients with a small or recessed chin may feel as if they don’t even “have” a chin because it is not prominent on their face. There is a way to fix this, whether by using implants or the patients own chin bone, to help the patient have the chin they desire. Cosmetic chin surgery is called genioplasty. This surgery can be performed based on the particular patient’s history, which usually includes previous orthodontic care and/or an overbite, but it can also be a purely cosmetic surgery. In this article, we will dive into plastic surgery options for a small or recessed chin, and explore what patients considering this type of procedure will need to know or prepare for during the recovery period. The two main types of procedures are Alloplastic Genioplasty and Horizontal Sliding Genioplasty. These procedures can be performed either individually or in conjunction with other facial cosmetic surgeries such as facelifts, neck lifts, and nose jobs (rhinoplasty). As explained more in depth below, the Alloplastic Genioplasty is the least invasive, and the Horizontal Sliding Genioplasty does require more recovery time after the operation.

Alloplastic Genioplasty

Alloplastic Genioplasty is a cosmetic solution to help alleviate the patient’s disproval of their small chin. This procedure in particular works by way of an implant being placed in the chin to alter the appearance. This is the best option for a patient who is concerned with the appearance of their chin, but who do not have any issues with where and how their teeth and jaws are. In other words, this is the option for those looking for a more cosmetic change rather than structural change. Implants available vary in size and shape, and the correct implant with the correct type of synthetic material will be decided in consultation with your doctor based on what you need. Your plastic surgeon will work with you to figure out how you would like the end result to look and will work with your facial profile and measurements to figure out the correct size to help you achieve the desired appearance. The results of this procedure will be individualized based on the facial aesthetics of the patient and the shape and size of chin they would prefer. The surgeon will then place the implant under the skin by way of either an incision in the mouth or an incision under the chin. Patients undergoing this procedure will be under local anesthesia or under sedation to eliminate discomfort during the procedure.

Recovery time from this procedure varies between individuals, but directly following the surgery the patient will need to wear a compression bandage around the chin to allow it to heal properly. It is normal to have some bruising and swelling for a little over a week after the procedure, and it is also important to note that it can take up to 3 months after the procedure is completed to see the full final results and to alleviate all of the bruising and swelling that may stick around after the procedure.

Horizontal Sliding Genioplasty

Unlike the Alloplastic Genioplasty, the Horizontal Sliding Genioplasty does not involve synthetic implants but instead uses the patient’s own bone to create the ideal chin appearance. This procedure consists of cutting the boney tip of the chin and moving it forward to help create a more prominent and desirable chin. Your surgeon then performs the process of making sure that the bone stays in place here. This is achieved through the use of screws and plates as necessary to ensure a solid end result. Perhaps obviously, this choice of treatment is much more invasive and requires a longer recovery time. It is essential you talk with your doctor to discuss which option may be best for you to help you get the look you desire. As with the other option, this procedure is performed under local anesthesia or while the patient is sedated.

Like the Alloplastic Genioplasty, the Horizontal Sliding Genioplasty also requires a dressing following the procedure to allow the chin to rest and heal and help to reduce swelling. The bruising and swelling should subside after around a week, but it is also normal to still have some aspects of these up to 3 months after the surgery when the chin is completely healed. This is also when the final surgery results will be visible. With Horizontal Sliding Genioplasty, because of the swelling and stretching of the tissue during the procedure, you may experience some numbness in the lip area after the surgery. This should be temporary, and if it is not it is important to talk to your doctor about a resolution.
